Nyssa Sports Physicals will be held at the Valley Family Medical in Nyssa this year at the following location:

The date and times remain the same as well as the ability to bring the form into the front office if you are unable to upload it to Final Forms. Please remember that ALL student athletes for both the high school and middle school will need to register with Final Forms to be able to participate in sports and you can do so at:
